ABSTRACT

Objective To explore the effect of propofol on pulmonary function ,inflammatory factors and postoperative pulmonary infection in elderly patients with esophageal carcinoma ,and to provide data for prevention and control of nosocomial infection.Methods From January 2011 to December 2015,155 elderly patients with esophageal cancer in Lishui Central Hospital were selected and randomly divided into control group (C group,n=70) and obser-vation group (O group,n=85) according to the digital table.In the maintenance of anesthesia ,O group was given propofol,C group was given sevoflurane.The alveolar arterial oxygen partial pressure difference ( PA -aO2), respiratory index(RI) and oxygenation index (OI) of the two groups were observed at different time points.The levels of IL-1,IL -6 and IL -10 in serum were measured at different time points ,and the pulmonary infections were observed after operation.Results At 30min,60min,90min after ventilation in the two groups ,the PA-aO2,RI were higher than the beginning of mechanical ventilation ,OI were lower than the beginning of mechanical ventilation (t=7.266,23.451,9.572,8.009,30.836,17.158,4.908,21.237,21.350,7.010,23.644,10.039,9.099,34.254, 16.679,8.217,35.434,22.160,all P<0.05).At 60min,90min after ventilation,the RI of O group were lower than those in C group(t=6.613,9.522,all P<0.05).The levels of IL -1,IL-6 and IL-10 in the two groups after opeation were higher than before operation (t=5.069,8.903,9.184,14.896,12.137,11.202,all P<0.05).The IL-1,IL-6 and IL-10 levels in O group after operation were lower than those in C group ,the IL-1 level at 6h postoperation in O group was lower than that in C group (t=10.442,4.803,2.513,4.076,all P<0.05).There were 13 cases of nosocomial pulmonary infection in all the 155 patients,the infection rate was 8.39%.There was no statis-tically significant difference in the infection rate between the two groups [6 cases(7.06%) in O group,and 7 cases (10%) in C group,χ2=0.432,P>0.05].Conclusion Elderly patients with esophageal cancer undergoing radical operation of propofol ,the effects of propofol on pulmonary function ,inflammatory factors are less,the incidence of postoperative pulmonary infection is low,it has good clinical application value.

ABSTRACT

Objective To analyze the clinical, endoscopic and histopathological features of eosinophilic gastroenteritis ( EG) in children. Methods A retrospective study of 76 children with EG was performed to analyze clinical symptoms, laboratory and imaging results, endoscopic and pathological features, status of Helicobacter pylori ( H. pylori) infection, treatment and outcomes. Results The main clinical symptoms were abdominal pain in 55. 3%(42/76) cases, vomiting in 39. 5% (30/76) cases and hematochezia in 38. 2% cases( 29/76) . The hemoglobin level decreased significantly in 34 cases ( 44. 7%, 34/76). Peripheral blood eosinophil (EOS) count increased significantly in 9 cases (11. 8%,9/76) and EOS percentage increased significantly in 13 cases(17. 1%,13/76). Total serum IgE elevated in 32 cases ( 54. 2%, 32/59 ) . There were also 18 cases ( 36. 7%, 18/49 ) positive in serum allergen?specific immunoglobulin E ( sIgE) test and 25 cases ( 32. 9%,25/76) positive in fecal occult blood test. Among 51 cases of abdominal ultrasound examination, there were 7 cases of ascites, 4 cases of pelvis fluid and 3 cases of intestinal wall change. Endoscopic examination in 76 cases showed 63 cases ( 82. 9%) of mucosal hyperemia/edema,20 cases ( 26. 3%) of ulceration, 17 cases ( 22. 4%) of erosion, 11 cases ( 14. 5%) of nodularity or hyperplasia and 9 cases ( 11. 8%) of normal mucosa. The pathological examination showed mucosal inflammation with a large number of EOS infiltration(≥20 per HPF).There were 12 cases(15. 8%, 12/76) of H. pylori infection. Among the 76 cases, clinical symptoms improved significantly in 74 patients after treatment with dietary allergen avoidance, anti?allergy medications, antacids, montelukast and corticosteroid, and the total efficacy was 97. 4%. The efficacy of dietary allergen avoidance, anti?allergy medications, antacids and montelukast was 93. 8%( 61/65 ) . The efficacy of corticosteroid was 86. 7%(13/15). Conclusion The clinical manifestations and endoscopic characteristics of EG in children lack specificity. In terms of diagnosis, the elevated total serum IgE and the positive sIgE test may be taken as reference for the diagnosis of EG. The definite diagnosis is based on pathological examination ( EOS infiltration≥20 per HPF).While in terms of treatment, dietary allergen exclusion, anti?allergy medications, antacids and montelukast are highly effective, which can be taken as the first option. There is no need of corticosteroid as routine therapy.

ABSTRACT

To carry out pathologic diagnoses and whole-genome sequence analyses of the Jaagsiekte sheep retrovirus (JSRV) in Xinjiang, China, we first observed sheep suspected to have the JSRV. Then, the extracted virus suspension was observed by transmission electron microscopy (TEM). Total RNAs from lungs of JSRV-infected sheep were extracted and reverse-transcribed using a cDNA synthesis kit. Six pairs of primers were designed according to the exogenous reference virus strain (AF105220). Reverse transcription-polymerase chain reaction was carried out from JSRV-infected tissue, and the whole genome of the JSRV sequenced. Our results showed: flow of nasal fluid ("wheelbarrow test"); different sizes of adenoma lesions in the lungs; papillary hyperplasia of alveolar epithelial cells; alveolar cavity filled with macrophages; dissolute nuclei in central lesions. TEM revealed JSRV particles with a diameter of 88 nm to 125. 4 nm. The full-length of the viral genome sequence was 7456 bp. BLAST analyses showed nucleotide homology of 96% and 95% compared with that of the representative strain from the USA (AF105220) and UK (AF357971). Nucleotide homology was 89.8% and 89.9% compared with the endogenous Jaagsiekte sheep retrovirus, Inner Mongolia strain (DQ838493) and USA strain (EF680300). The specific pathogenic amino-acid sequence "YXXM" was found in the TM district, similar to the exogenous JSRV: this gene has been reported to be oncogenic. This is the first report of the complete genomic sequence of the exogenous JSRV from Xinjiang, and could lay the foundation for study of the biological characteristics and pathogenic mechanisms of the pulmonary adenomatosis virus in sheep.

ABSTRACT

An improved method for the determination of Vitamin D2 in baby food (fortified dried milk) was established by using two steps of high-performance liquid chromatography (HPLC). The sample was directly saponified and extracted with benzene. The extracted unsaponifiable matter was firstly subjected to preparative HPLC using a Nucleosil 5C18 column (reversed-phase type) with acetonitrile-methanol (3:2) as a mobile phase. This first step was for the purpose of clean-up, and a fraction containing Vitamin D2 was collected. The separate fraction was subsequently subjected to analytic HPLC with a Zorbax SIL column (straight-phase type) with isopropanol-hexane (0.8 : 99.2) as a mobile phase. The Vitamin D2 was separated from other contaminants and determined with ultraviolet spectrograph by the peak height estimated. The recovery rate was 84%, standard deviation 12.57 and coefficient of variation 4.57%. The proposed method was rapid, accurate, and suitable for the determination of baby food.
